Selected and compiled by Survivors Registry staff and volunteers from the USHMM Archives microfilm collection "Ustashas Supervisory Office (UNS), Zagreb, records, 1941-1943" (Accession Number: 1998.A.0018, reel 2). Original records located in Croatian State Archives in Zagreb [= Hrvatski drzavni arhiv u Zagrebu] as record group 2.1.1. The United States Holocaust Memorial Museum Archives selected the records for filming in 1997 and received the microfilm reels on Feb. 13, 1998.

Contains various official government correspondence to and from the Independent State of Croatia's Ministry of the Interior, Directorate of Public Order and Security in Zagreb [In Serbo-Croatian: "Nezavisna Drzava Hrvatska, Ministarstvo Unutarnjih Poslova. Ravnateljstvo za javni Red i Sigurnost u Zagrebu] with at least 21 non-alphabetical name registers of Jews, primarily from Zagreb, Croatia, who had converted to various Christian denominations or to Islam along with examples of requests for exemption from the requirement to wear a Jewish Star instituted in Croatia from June 4, 1941.
